A fire door only works if it closes, latches, and holds back fire and smoke long enough for people to get out. That’s the whole job. When a fire door fails inspection, it’s usually because something small got ignored for months — a gap that grew, a label that got painted over, a closer that stopped pulling the door shut all the way.
NFPA 80, the Standard for Fire Doors and Other Opening Protectives, requires an annual inspection and test for every fire door assembly in a commercial building NFPA, “Fire Doors and NFPA 80 FAQs,” 2025. Doors also get inspected right after installation, before they ever see daily use NFPA, 2025. If you manage a commercial building in Broward County, this inspection isn’t optional and it isn’t a formality. Fire marshals check for it, and insurance carriers ask about it after a loss.
This article covers what the inspection actually looks at, what fails most often on real buildings, and how to fix problems before an inspector finds them.
What NFPA 80 Requires
NFPA 80 requires that a qualified person — someone who understands the door’s operating components and the type of fire door being tested — perform the inspection NFPA, 2025. That inspector can be an employee of the building owner or a third party, as long as the AHJ (authority having jurisdiction — usually the local fire marshal) accepts their credentials NFPA, 2025.
The standard lists a series of items that must be checked on every fire door assembly during the annual inspection. According to NFPA’s own summary of the standard, inspectors confirm NFPA, 2025:
- Labels are visible and legible
- No damage to the door, frame, or hardware
- No broken or missing parts
- Clearances around the door are within limits
- Any signage on the door is code-compliant
- The door closes and latches fully under an operational test
The door must stay operable at all times — it has to open, close, and latch without assistance NFPA, 2025. Wedging a fire door open, propping it with a chair, or disabling a closer defeats the entire assembly, and NFPA 80 prohibits it outright NFPA, 2025.
What Actually Fails on Real Buildings
We inspect and service fire door hardware on commercial buildings across Hollywood, Fort Lauderdale, and Pembroke Pines. The same handful of problems show up over and over.
Clearance gaps that grew over time
Every fire door assembly has an allowed clearance range around the door — the gap between the door and the frame, and the gap at the bottom. Doors settle. Hinges wear. Frames shift slightly after years of use. A gap that started within spec can widen past the allowed limit without anyone noticing, because nobody measures it day to day.
Once a gap is out of tolerance, smoke and flame can pass around the door faster than the rating assumes. The fix is usually adjustment — resetting hinges, shimming the frame, or in worse cases replacing a warped door.
Missing or painted-over labels
The label on a fire door is not decoration. It’s the proof that the door, frame, or window was tested and listed by a certification agency acceptable to the AHJ NFPA, 2025. The label has to show the manufacturer’s name, the certification agency’s mark, the door’s rating, the fire test standard it was tested to, and in some cases the temperature rise data from the listing test NFPA, 2025.
Painting crews repaint hallways and stairwells all the time, and fire door labels get hit along with everything else. NFPA 80 requires labels to stay legible and prohibits removing, defacing, or obscuring them while the door is in service NFPA, 2025. A label can technically be painted over and still pass if it remains readily visible and legible — for instance, an embossed label where the raised lettering still reads through a thin coat — but NFPA itself advises against painting fire door labels at all NFPA, 2025. In practice, most painted-over labels fail inspection because the lettering is no longer readable.
If a label is missing or illegible and can’t be salvaged, the rating isn’t automatically void — NFPA 80 allows the rating to be verified through other means the AHJ accepts, like a field inspection service or documentation from a certification service NFPA, 2025. That process takes time and costs money. It’s cheaper to protect the label in the first place — tell painting crews to mask it, or replace the door if the label is already gone.
Damaged gaskets and seals
Smoke seals and intumescent gaskets around the door frame are part of the listed assembly. When they crack, peel, or get torn off during moves and deliveries, the door may still close and latch fine — but the smoke-sealing function is compromised. This kind of damage is easy to miss on a quick walk-through because the door looks fine until you look at the frame edge closely.
Failed latch engagement
A fire door has to latch, not just close. Doors get slammed, closers get knocked out of adjustment, strikes wear down, and eventually the latch stops fully engaging — the door looks shut but a firm push opens it. This is one of the most common fails during an actual operational test, because closer adjustment drifts gradually and nobody notices until someone tests it under real conditions.
Fire Door Ratings
Fire doors carry a rating in minutes — the length of time the assembly is designed to hold back fire in a listing test. Common ratings on commercial buildings include 20, 45, 60, and 90 minutes, and the required rating for a given opening depends on the fire resistance rating of the wall or partition the door sits in NFPA, 2025. To determine what rating a specific opening needs, NFPA 80 points to tables in codes like NFPA 101 that cross-reference the wall assembly’s rating to the required opening protection NFPA, 2025. A stairwell door in a high-rise and a corridor door in a strip mall office won’t necessarily carry the same rating, even if they look similar.
What Happens If a Door Fails Inspection
What is clear: NFPA 80 requires that deficiencies found during inspection get corrected without delay, and the door has to remain operable and code-compliant in the meantime. A building that can’t produce inspection records, or that has known deficiencies left unaddressed, is exposed if a fire marshal does a walk-through or if there’s ever a fire and an insurance investigation follows.
Getting Ready for Inspection
The best time to fix a fire door problem is before the inspector shows up. A few things a building manager can check between annual inspections:
- Walk hallways and stairwells and look at every door label — is it visible, legible, not painted over?
- Check that doors close fully and latch on their own without being pushed
- Look at the gap around each door for obvious daylight or unevenness
- Confirm nothing is wedging or propping a fire door open
- Check gaskets and seals along the frame for tears or gaps
If any of that turns up a problem, get it corrected before the annual inspection date, not after.
